Coach Certification Course

I certify that the information provided above is true and complete. I understand that additional documentation may be required upon request. I agree to comply with all USAWKF rules and policies and acknowledge that providing false or misleading information may result in the nullification of my certification exam and certified coach levvel.

By registering for and participating in the USAWKF Coach Certification Training, I acknowledge and agree to the following:

1. Voluntary Participation
I understand that participation in this training is voluntary and that course involves physical activity that carries an inherent risk of injury. I choose to participate at my own risk.
2. Health and Safety
I certify that I am physically able to participate in this training. I will practice within my own abilities, stop immediately if I experience pain, dizziness, or discomfort, and seek medical advice when appropriate.
3. Release of Liability
To the fullest extent permitted by applicable law, I release and hold harmless the United States of America Wushu Kungfu Federation (USAWKF), its officers, instructors, volunteers, and organizers from any claims, liabilities, or damages arising from my voluntary participation in this training, except where prohibited by law.
4. Course Materials
I understand that all course materials, recordings, and instructional content are the intellectual property of USAWKF or the respective instructors and are provided for my personal educational use only. I will not reproduce, distribute, or publicly share these materials without prior written permission.
5. Certification Requirements
I understand that participation in this training does not, by itself, guarantee coach certification. Certification is granted only after all applicable USAWKF requirements have been successfully completed.
